Process & Form
Nominations should be submitted via the Spot Award Form. Nominations will be accepted throughout the fiscal year and awards will be processed regularly based on each campus control unit timing. Nominations must be submitted within three months of the accomplishment.
- The supervisor's and next level manager's signatures on the Spot Award Form represent an endorsement of the nomination.
- Completed nomination forms are submitted to the designated Recognition Award Administrator for each control unit.
- The Recognition Award Administrator reviews the nomination and notifies the nominator that the award nomination:
- is approved (see #4 below).
- is denied.
- If the award is approved, the Recognition Award Administrator provides an award letter template to the nominee’s supervisor to be presented to the recipient. At the time the award letter is presented, a one-time payment should be processed via OPTRS to be paid in the next month.
- Awardees receive an award letter and a deposit of the $250 Spot Award (less taxes) in a future paycheck.
Note: If the nominator is not the supervisor or manager OR the nomination is for an employee in another control unit or division, the completed nomination form should be submitted to the Recognition Award Administrator for that area. This individual can contact the employee’s supervisor so the nomination can be reviewed for eligibility.
